Report on Caregiver Incident Form

Life is full of unknowns, and unexpected events can occur at the drop of a dime, altering our lives in a variety of ways.

As a Foster Parent, when unexpected events happen, you are contractually required to report these changes to your Resource Social Worker.

Sometimes these occurrences are positive, and sometimes they are negative. Below are six possible examples:

  1. You\’ve been planning on starting your family in year five of your marriage, and that time has finally arrived. You\’re going to have a baby! Congratulations!
  2. You and your spouse have waited for years; your dream of building your own home has finally come true. In your mind, you know exactly what your dream home will look like. You have architects to hire, contractors and landscapers to meet with. As thrilling as this venture is, it can also open the door to huge adjustments within the home.
  3. The cost of living has skyrocketed, and for young people, it\’s difficult to afford a place of their own. One of your biological children has had to move back home.
  4. Your parents are getting older and in need of additional care. Balancing life as a Foster Parent, spouse, and parental caregiver will impact everyone in the household.
  5. Foster Parenting has put a strain on your marriage. You decide to separate for six months and see how things go.
  6. The death of a family member or close friend.

Regardless of what changes take place within your home or within your extended family, it is imperative you discuss your situation with your Resource Worker, fill in the Report on Caregiver Incident Form and ensure a copy is added to your Resource file.
